Fees for services
BLOOM Therapy and Wellness, LLC is a private-pay, out of network provider. If requested, BLOOM can provide caregivers with a superbill that they can then submit to their insurance company for possible reimbursement, however, reimbursement is not guaranteed. A good faith estimate will be given prior to the start of services.
Referrals
Wisconsin is a direct access state which means that you do not need a referral from your primary care physician for an evaluation and treatment by an occupational therapist. The decision to obtain OT services is made by you. Please note that a referral may be needed when submitting a superbill. Please inquire with your insurance carrier.

How can I pay for services?
We require payment and a credit card on file to initiate services. We accept cash, check, credit card or Venmo forms of payment. Occupational therapy is HSA and FSA eligible. Upon request, we can provide you with a superbill that can be submitted to your insurance carrier for possible reimbursement. This reimbursement is not guaranteed.

Our primary focus is on the needs of your child. Being a private pay provider has many benefits to you as the consumer.

Do you provide other therapies?
At this time, BLOOM Therapy and Wellness, LLC provides only pediatric occupational therapy services. Please check back frequently to inquire if additional services are available.

Are you licensed?
Our occupational therapist is licensed in both Wisconsin and Illinois. She is board certified by the National Board for Certification in Occupational Therapy.
